( effective June 1, 1997 )

Personal Watercraft (Motorized) will only be allowed to be used at Middle Florida Baptist Assembly if the following conditions are met:

1. The Owner / Agent of said Watercraft are present and have completed a Watercraft Release Form and presented it to the Manager of the Assembly.

2. All persons operating or using said Watercraft have completed a Watercraft Release Form and presented it to the Manager of the Assembly.

3. All persons operating said Watercraft are above the age of 16 and possess a valid Drivers License and have presented it to the Manager of the Assembly.

4. Said Watercraft may only be operated between the hours of 9:00 AM and 6:00 PM.

5. Said Watercraft may not be operated within 50 yards of the Swimming Area.

6. All persons operating or using said Watercraft must wear U.S. Coast Guard approved life vests, follow all general boating safety rules and follow all Florida boating laws.

*** Failure to meet all of the above conditions will result in the removal of all the said Watercraft from the Assembly. ***

Watercraft Release Form

I,________________________________(signature of Parent or Guardian of Operator or Participant), do hereby agree to follow the above policies and do hereby release and forever discharge all sponsors, Middle Florida Baptist Assembly; Inc., and their employees from any and all claims, demands, actions or cause of action, past, present or future arising out of any damage or injury as a result of the use and operation of said Watercraft.

I,________________________________(signature of Operator or Participant), do hereby agree to follow the above policies and do hereby release and forever discharge all sponsors, Middle Florida Baptist Assembly; Inc., and their employees from any and all claims, demands, actions or cause of action, past, present or future arising out of any damage or injury as a result of the use and operation of said Watercraft.

I,________________________________(signature of Watercraft Owner / Agent), do hereby agree to follow the above policies and assume responsibility for the use of said Watercraft and do hereby release and forever discharge all sponsors, Middle Florida Baptist Assembly; Inc., and their employees from any and all claims, demands, actions or cause of action, past, present or future arising out of any damage or injury as a result of the use and operation of said Watercraft.
